LICENSED PRACTICAL NURSE (LPN)

Summary

This is a valuable position that helps promote and maintain the optimal health of individuals served through assessment, documentation and teaching. Serves as a liaison to physicians and other health care professionals and provides on-going medical in-service training to staff.

Pay: $30.00 – $32.50 per hour
Expected hours: No less than 40 per week

Essential Duties & Responsibilities
  • Participate in the development, review and monitoring of the medical section of the Individualized Support Plan for all individuals served.
  • Attend and participate in supported living Individual Support Plan (ISP) meetings.
  • Monitor & review the medication administration records for supported living sites.
  • Conduct regular visits to the homes and apartments to monitor the health of individuals served.
  • Review status of medication storage and administration, first aid supplies, incident reports and complete designated nursing reports and medical paperwork.
  • Able to travel to all supported living sites throughout Northwest Indiana.
  • Participate in pre-admission evaluations and development of discharge plans when appropriate.
  • Provide in-service training for individuals served, and staff as needed.
  • Able to provide physical assessments of individuals on a regular basis.
  • Serve as a liaison to physicians and other health care professionals when requested for all programs.
  • Monitor and assist with any chronic or acute medical issues for individuals in all programs when requested.
  • Be available during non-work hours for phone calls regarding medical emergencies and urgent medical situations, assisting with follow-up if needed in a timely manner.
  • Conduct Medication Administration training for Med Core A and B as necessary.
Qualifications

Must hold a Licensed Practical Nurse’s certification in the State of Indiana and have one years’ experience in a clinical setting.
Not convicted of an abuse, neglect or position relevant felony.
Current valid driver’s license

Interpersonal skills including an ability to work with individuals with developmental disabilities.
  • Ability to maintain strict confidentiality of individual’s information.
  • Ability to maintain scheduled hours.
  • Familiarity with state and federal regulations.
  • Ability to learn keyboarding and computer skills.
  • Ability to follow plans that address a consumer’s needs.
  • Ability to teach the Medication Administration curriculum.
