What is Case Management?

The Indiana Bureau of Developmental Disabilities Services (BDDS) states that:
Case Managers are to monitor service delivery and utilization. Case Managers are to ensure quality of care by case reviews which focus on the individual's progress in meeting goals and objectives established through service plans. Case Managers are to monitor and document the quality, timeliness and appropriateness of the care, services and products delivered to an individual. Case Managers must be vigilant in our oversight of service programs as well as the protection and success of the consumers we serve. Case Managers provide another level of oversight to assure consumers are healthy, safe and successful in the community.
